Production
Background
Extreme Rules is an annual gimmick pay-per-view (PPV) and WWE Network event produced by WWE since 2009. The concept of the show is that the event features various matches that are contested under hardcore rules and generally features one Extreme Rules match. The defunct Extreme Championship Wrestling promotion, which WWE acquired in 2003, originally used the "extreme rules" term to describe the regulations for all of its matches; WWE adopted the term and has since used it in place of "hardcore match" or "hardcore rules." The 2020 event was titled as The Horror Show at Extreme Rules, but the title returned to "Extreme Rules" for 2021. The 2021 event will be the thirteenth event under the Extreme Rules chronology and feature wrestlers from the Raw and SmackDown brands.
Storylines
After Gunther successfully defended the Intercontinental Championship against Sheamus at Clash at the Castle, Imperium (Gunther, Giovanni Vinci and Ludwig Kaiser) defeated The Brawling Brutes (Sheamus, Butch and Ridge Holland) in a Six Man Tag Team Match on the September 9, 2022 episode of SmackDown. The following week, Butch and Holland won a Fatal Four Way Tag Team Match, that involved Vinci and Kaiser, to earn a match for the Raw Tag Team Championship and SmackDown Tag Team Championship. However, Butch and Holland failed to win their earned championship match after Imperium attacked Sheamus at ringside. As a result, a Good Old Fashioned Donnybrook Match was schedulded between The Brawling Brutes and Imperium for Extreme Rules.
At WrestleMania 38, Edge formed The Judgment Day with Damian Priest and Rhea Ripley. Finn Bálor would also join the group, however, Bálor would take leadership after he, Priest, and Ripley turned on Edge. At Clash at the Castle, after Edge and Rey Mysterio defeated Bálor and Priest, Rey's son, Dominik, turned on him and Edge, also joining The Judgment Day. On the September 12, 2022 episode of Raw, Edge faced Dominik, however, The Judgment Day would ambush Edge, attacking him with a chair. On the September 26 episode of Raw, Edge made his return, challenging Bálor to an "I Quit" Match at Extreme Rules, which Bálor accepted.
At Money in the Bank, Liv Morgan cashed in her Money in the Bank briefcase to win the SmackDown Women's Championship from Ronda Rousey. In a rematch at SummerSlam, Morgan successfully retained the title in controversial fashion, pinning Rousey despite having tapped out from an armbar before the three count, something the referee failed to see. Rousey attacked Morgan and the referee after the match and was consequently suspended. On the September 9, 2022 episode of SmackDown, with Rousey's suspension having concluded, she won a Fatal Five Way Elimination Match to become the #1 Contender for the SmackDown Women's Championship. The following week, Morgan challenged Rousey to an Extreme Rules Match, which she accepted.
At SummerSlam, Bianca Belair defeated Becky Lynch to retain the Raw Women's Championship. After the bout, Belair was confronted by a returning Bayley, who was joined by Dakota Kai and IYO SKY, a trio that would be dubbed Damage CTRL. Belair, joining forces with Alexa Bliss and Asuka, faced Damage CTRL in a Six Woman Tag Team Match at Clash at the Castle. Bayley would pin Belair to win the match. On the September 26, 2022 episode of Raw, Bayley, restating she had pinned Belair at Clash at the Castle, challenged Belair to a Ladder Match at Extreme Rules, which Belair accepted.
At Clash at the Castle, Matt Riddle was defeated by Seth "Freakin" Rollins. Rollins would go on to face Bobby Lashley for the WWE United States Championship on the September 19, 2022 episode of Raw. However, Rollins would be defeated after Riddle distracted Rollins. Later in the night, Riddle challenged Rollins to a Fight Pit Match at Extreme Rules, which Rollins accepted.
On the August 5, 2022 episode of SmackDown, Karrion Kross, along with Scarlett, made their return to WWE, with Kross attacking Drew McIntyre. Kross would continue to target McIntyre over the following weeks, fleeing before McIntyre could fight back. On the September 23 episode of SmackDown, McIntyre announced that WWE management had granted him a Strap Match with Kross at Extreme Rules to ensure Kross would be unable to flee.
